Terms and Conditions & Consent Agreement 

Telehealth Informed Consent for The Beyond Bariatrics Membership (BBM).

I understand that my health and wellness provider "The Dietologist" wishes me to access educational video modules and Monthly Group Coaching Calls via Zoom for a minimum of 6 months as part of the Beyond Bariatrics Membership.

This means that through an interactive video connection, I will be able to access monthly group support with The Dietologist about my health and wellness concerns. NB other clients are invited to the monthly coaching calls. You can keep your identity confidential during the calls, if you wish, by switching the camera off. 

I understand there are potential risks with this technology: 

The video connection may not work or it may stop working during the consultation. The video picture or information transmitted may not be clear enough to be useful for the consultation. 

The benefits of a telehealth (Zoom) group coaching calls are: 

• I do not need to travel to the consult location. 
• I have access to a specialist dietitian/emotional eating coach. 
• I can share my experience and learn from other members.

For the duration of your support you will have access to a PRIVATE FACEBOOK COMMUNITY,  BEYOND BARIATRICS with 24hr access to an online educational video resource library via the platform MEMBER VAULT, providing a step by step, SELF PACED approach to working with your surgery and managing emotional eating/habits. It is in your best interest to complete the modules and worksheets (homework) on this self paced membership and attend monthly Q&A sessions via zoom for more personalised support from Van (The Dietologist)

If at any point you are struggling, you MUST reach out to Van (The Dietologist) via email enquiries@thedietologist.co.uk or the FACEBOOK community, asking for help with a full description of your concerns. If I don’t hear from you I will assume that you are working through the program. From time to time I may reach out to you IF you do not attend coaching calls. If you want to upgrade your support i.e. ad hoc one to one coaching please discuss this with The Dietologist.

   Please add the monthly coaching session dates  (1st Tuesday of Every Month at 5pm) to your calendar.

If you are unable to attend please submit your questions a day in advance in the Beyond Bariatrics Facebook group. 

A recording of each Q&A session will be available to watch 24hrs after the session. Occasionally there can be technology issues where the sessions may not be recorded and unfortunately this will be out of my control, sorry.

Cancellation & refund Policy (virtual and BBM ) . 

We operate a no refunds policy after purchasing a health program.

Payment Plan Clients ONLY:  

You have agreed to pay the full fee in installments. The monthly payments will be automatically debited from your  account, after the initial payment and invoice raised by stripe. You MUST complete all payments and cannot  walk away from your payments or treatments. If you decide to walk away, you remain fully responsible for the full  cost of your support package and any remaining installment  payments. 

If you require ad hoc consultations, one to one nutrition assessments, GP letters or a review of your blood tests you will need to contact The Dietologist via email enquiries@thedietologist.co.uk where we will send you a price list for what you require. I acknowledge that the dietary advice via the video platform and zoom Q & A sessions is based on the most up-to-date evidence to achieve my desired health goal, however no guarantees have been made to me as results may vary based on your individual response and adherence to dietary advice and support.

The Dietologist may wish to send you relevant resources as part of your dietetic treatment.*

Further information on how we use your personal information can be found in our Privacy Policy and Terms & Conditions. Please ensure you have read these before signing this consent form. You can change your  preferences at any point by contacting The Dietologist at enquiries@thedietologist.co.uk. If you no longer wish to  hear from The Dietologist, please contact enquiries@thedietologist.co.uk to unsubscribe from communications.*
